Treatment That Accepts UnitedHealthcare

UnitedHealthcare Plans Leora Accepts
Leora accepts UnitedHealthcare plans you get through an employer and the Individual and Family plans UnitedHealthcare sells in Ohio. It also accepts UnitedHealthcare Community Plan of Ohio, which is an Ohio Medicaid plan: see Treatment with Ohio Medicaid.
Does UnitedHealthcare Cover Rehab?
It depends on your plan. UnitedHealthcare says coverage depends on the specific plan, and that not every plan includes behavioral health benefits.
For many UnitedHealthcare members, mental health and substance use benefits are managed by United Behavioral Health, a subsidiary of Optum, which administers them. Optum makes its coverage decisions for substance use treatment with the ASAM Criteria, Fourth Edition, from the American Society of Addiction Medicine.
A federal law, the Mental Health Parity and Addiction Equity Act, generally prevents group health plans and insurers that provide mental health or substance use benefits from putting less favorable limits on them than on medical and surgical benefits. Every Marketplace plan covers mental health and substance use services as essential health benefits.
Prior Authorization for IOP and PHP
Optum's prior authorization list for UnitedHealthcare commercial plans includes intensive outpatient and partial hospitalization. The 2026 list for UnitedHealthcare's Individual and Family plans in Ohio names the same services. According to Optum's provider manual, most plans do not require prior authorization for routine outpatient therapy. Requirements differ from plan to plan.
Sublocade and Brixadi
UnitedHealthcare's commercial drug policy lists the buprenorphine injections Sublocade and Brixadi as proven for moderate to severe opioid use disorder for patients who meet its criteria, with each authorization lasting up to 12 months. Whether your plan covers them depends on your plan. Leora's MAT program uses both.
Checking Your Benefits
Call the mental health number on your UnitedHealthcare member ID card, or sign in to your member account or the UHC app. Optum's member site, liveandworkwell.com, also shows behavioral health benefits.

Every Leora program is outpatient; there is no residential or inpatient care. Whether a program is covered for you depends on your assessment and your plan.
