Ohio Medicaid Plans Leora Accepts

Ohio Medicaid has seven managed care plans, and Leora accepts all of them:

• AmeriHealth Caritas Ohio
• Anthem Blue Cross and Blue Shield
• Buckeye Community Health Plan
• CareSource Ohio
• Humana Healthy Horizons in Ohio
• Molina Healthcare of Ohio
• UnitedHealthcare Community Plan of Ohio

Your plan's name is on your member ID card.

What Ohio Medicaid Covers

Ohio Medicaid pays for substance use disorder treatment by level of care, following the American Society of Addiction Medicine (ASAM) criteria. The state's rule lists outpatient services, and intensive outpatient and partial hospitalization, including withdrawal management at that level.

Mental health treatment is covered too: under the state's rule, Medicaid pays for behavioral health services that are medically necessary, for a diagnosed mental illness or substance use disorder.

Whether a program is covered for you depends on your assessment and your plan. Every Leora program is outpatient; there is no residential or inpatient care. Programs at Leora:

• Outpatient Withdrawal Management (Ambulatory Detox)
• Partial Hospitalization (PHP)
• Intensive Outpatient (IOP), in person at both facilities
• Virtual IOP
• Outpatient
• Medication Management and MAT
• Case Management

Prior Authorization for Partial Hospitalization

Ohio's rule requires prior authorization for partial hospitalization, which it sets at a minimum of 20 hours of services a week.

Checking Your Coverage

Have your Medicaid member ID card with you when you call admissions. Admissions can check your coverage while you are on the phone. This website never asks for your insurance details.

If You Do Not Have Medicaid Yet

You can apply online at Benefits.Ohio.Gov, or by phone through the Ohio Medicaid Consumer Hotline at 800-324-8680, whose staff can help with your application.

You can also apply in person or by mail at your county Department of Job and Family Services. For help completing an application, the state lists 844-640-6446.

Changing Your Medicaid Plan

You can change plans in the first 90 days after you join one, and every year during open enrollment, November 1 to November 30. The change takes effect on the first day of the month after you choose. At other times you can ask to change for just cause, such as trouble getting the care you need.

Getting to Appointments

If you have full Medicaid and have trouble getting to a medically necessary service, you can ask for help with transportation. What is available depends on your plan and the county you live in.
